Physical Therapist – Mobile Outpatient Therapy

Provide in-home outpatient physical therapy for older adults, delivering evaluation, treatment, and ongoing rehabilitation in patients’ homes or in independent living facilities. Develop and implement individualized physical therapy programs to restore independence, educate and train patients and caregivers, and refer to other disciplines as needed. Supervise Physical Therapy Assistants and students when applicable; ensure adherence to schedules and thorough clinical and billing documentation; participate in team conferences; perform home and community mobility assessments; and design treatment plans that address functional goals. This full-time role (35 hours/week) requires a valid physical therapy license and a Master’s degree in Physical Therapy (or equivalent combination of degrees/experience). The position supports H-1B sponsorship and relocation assistance; opportunities to work across the U.S. with travel as needed.
