Job Summary
Mobile in-home outpatient Physical Therapist providing evaluation and treatment for older adults in patients’ homes or in independent living facilities. Develops and delivers individualized physical therapy programs to restore, reinforce, and enhance functional performance; educates patients and caregivers; collaborates with rehabilitation team; supervises PT assistants and students as needed; ensures timely documentation and participates in care conferences; travels to patient residences to deliver services and supports safe home mobility.
Required Qualifications
- Valid Physical Therapy license in state(s) of practice
- Master’s degree in Physical Therapy or Master’s in Physical Therapy related field with Bachelor’s in Physical Therapy or Bachelor’s in Physical Therapy plus 5 years progressive PT experience
