Job Summary
Evaluate and treat older adults in home and independent living facility settings, developing and delivering individualized physical therapy programs to restore function and promote independence. Screen, examine, and assess patients to establish diagnoses, identify impairments, and design comprehensive plans of care including therapeutic exercise, manual therapy, and adaptive device training. Instruct patients and caregivers on mobility skills and safety, while making referrals to other disciplines to address holistic patient needs. Supervise therapy assistants and support personnel as required, ensuring adherence to clinical documentation, billing standards, and department schedules. Maintain a safe environment by monitoring fire hazards and managing equipment needs.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
