Job Summary
Provide outpatient rehabilitation services in an in-home setting for the older adult population, evaluating and treating patients to facilitate rehabilitation in their private residences or independent living facilities. Assess patient needs, develop individualized therapy programs, and instruct patients and caregivers in skills essential for promoting independence. Screen, examine, and evaluate patients to establish diagnoses, design treatment plans, and implement therapeutic techniques including exercise, manual therapy, and functional training. Supervise Physical Therapy Assistants, temporary licensees, and students while adhering to department schedules and documentation requirements. Consult with the rehabilitation team to select appropriate regimens, order supplies, and ensure a safe environment free of hazards.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- Valid driver's license or reliable, independent transportation to and from the patients' residence
- Must be able to lift 50 lbs
