Job Summary
Provide outpatient rehabilitation services in an in-home setting for the older adult population, evaluating and treating patients to facilitate rehabilitation. Assess patient needs, develop individualized therapy programs, and instruct patients and caregivers in skills essential for promoting independence. Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel while adhering to department schedules and documentation requirements. Consult with the rehabilitation team to select therapeutic regimens, adapt plans based on individual needs, and make referrals to other disciplines. Perform home and community mobility assessments, order supplies, maintain equipment, and promote a safe environment by eliminating hazards.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
- Must be able to lift 50 lbs
