Job Summary
Conduct outpatient physical therapy services in patients' private residences or independent living facility rooms, evaluating needs, developing programs, and delivering treatments to restore function and promote independence. Direct activities to maintain health, instruct patients and caregivers on essential skills, and perform home and community mobility assessments to ensure safe movement. Supervise therapy assistants and students, consult with the rehabilitation team to select therapeutic regimens, and adapt plans based on individual patient progress. Manage equipment maintenance, order supplies, and complete clinical and billing documentation while adhering to department schedules. Maintain a safe environment by identifying hazards and enforcing safety protocols.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- Valid driver's license or reliable, independent transportation to and from the patients' residence
- Ability to supervise Physical Therapy Assistants, temporary licensees, students and support personnel in accordance with state licensure requirements and professional standards
- Adherence to department and patient treatment schedule, timely clinical and billing documentation requirements as well as work related meetings
- Attendance at and contribution to patient care, staffing conferences and other related meetings
- Consultation with other members of the rehabilitation team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient
- Adaptation of program and/or plan of care according to the needs of the individual patient
- Promotion of a safe environment and enforcement of the elimination of fire and safety hazards
- Ordering of supplies and equipment as necessary
- Cleaning, maintenance, identification, and reporting of the need for repair of equipment as necessary
- Screening, examination and evaluation of patients, including history, systems review, and application of appropriate tests and measures
- Synthesis of information and interpretation of examination findings to establish a diagnosis, identify impairments, determine the predicted level of improvement and the time required to achieve it
- Identification of precautions/contraindications
- Design and planning of a plan of care
- Development of appropriate treatment goals and methods in collaboration with the patient and caregivers
- Implementation of the physical therapy treatment plan
- Completion of all related documentation and record keeping regarding these services
- Application of therapeutic techniques including aerobic capacity conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility exercises, gait and locomotion training, neuromotor development training, relaxation, strength, power, and endurance training
- Application of functional training
- Application of manual therapy
- Application of airway clearance techniques
- Application of integumentary repair and protection
- Use of electrotherapeutic, physical agent and mechanical modalities
- Education on Chronic Disease Management
- Falls Assessment and Interventions to reduce fall risk
- Case Management in the home and community
- Design, fabrication, fitting and/or application of adaptive devices
- Training of patients in the use of adaptive devices including orthotics and prosthetics
- Selection of appropriate activities suited to the individual's physical and mental capacity, intelligence level and interest
- Instruction of patients in the skills and techniques required for participation in mobility activities
- Evaluation of patient progress, attitudes, and behavior as related to rehabilitative potential
- Performance of routine reexamination as needed/required to modify/progress plan of treatment
- Incorporation of health and wellness programming into treatment plans for patients
- Performance of home and community mobility assessments and provision of recommendations for safe mobility in the home and community space
