Job Summary
Develop a patient-centered plan of care incorporating safety, educational, and developmental needs while interpreting assessment data to set and revise goals. Administer therapeutic procedures and modalities under medical prescription, addressing home exercise programs and explaining treatment outcomes to patients and families. Maintain an adequate caseload adhering to productivity standards, comply with documentation requirements, and communicate regularly with supervisors, team members, and referring physicians. Present educational programs to Rehab staff and attend continuing education seminars as required.
Required Qualifications
- Graduate of an accredited School of Physical Therapy
- Licensed or eligible for licensure to practice in the State of Connecticut
Desired Qualifications
- Experience with electronic medical charting
- 3-5 years clinical experience
- Flexibility with scheduling
