Job Summary
Physical Therapist (PT) to provide comprehensive skilled therapy in patients’ homes as ordered by the attending physician, including evaluating and developing a plan of care, educating patients and families, and ensuring compliant documentation. Travel to patient locations is required; role involves OASIS assessments, supervision of PTAs/nurse aides as appropriate, and coordination with an interdisciplinary team to promote recovery and reduce hospital readmissions. Requires active PT license in state of employment, valid driver’s license/auto insurance, and strong communication and clinical judgment skills. Travel-heavy home health environment with flexible scheduling and potential bonuses.
Required Qualifications
- Physical Therapist with a current license in the state of employment
- Minimum of one year of experience as a PT in an acute setting is required
