Job Summary
Provide comprehensive skilled physical therapy in patients' homes as ordered by the attending physician. Conduct OASIS assessments and develop individualized plans of care; educate patients and families; document thoroughly per Medicare/Federal guidelines; supervise PTAs when appropriate; travel to patient locations; engage in care coordination to reduce hospital readmissions and support transitions home.
Required Qualifications
- Current Physical Therapist license in the state of employment
- Minimum of one year of experience as a PT in an acute setting
- Home care experience desired
- Valid driver’s license and auto insurance in applicant’s name
