Job Summary
Plan and administer prescribed physical therapy treatment for patients in their homes to attain highest level of physical function. Assess and document patients' physical strengths to support physician evaluation and guide treatment goals. Develop and implement rehabilitation programs aligned with physician Plans of Treatment. Provide oversight and clear written care instructions to PTAs and Home Health Aides, document observations and evaluations promptly, and report patient progress to the physician and care team. Requires an active Physical Therapy license and one year of PT experience; includes participation in Humana’s driver-safety program and a 40-hour workweek base for full-time employment. Located in Lafayette, LA.
Required Qualifications
- Current and unrestricted Physical Therapy license
- One year of experience as a Physical Therapist
- Experience collaborating with a team of healthcare professionals
- Driver's license and vehicle insurance (per the driver-safety program)
