Physical Therapist

Collaborate with therapy teams and the Rehab Department to complete evaluations, documentation, and recommendations supporting patient communications. Deliver treatments aligned with established plans to improve patient abilities and challenges while independently managing workflow for individual and group treatment notes in the electronic records system. Ensure therapy aligns with Medicaid and Medicare billing standards and understand PDPM guidelines. Requires a New Jersey license and background in long-term care within a Skilled Nursing Facility.
