Job Summary
Provide one-on-one physical therapy to older adults in private residences and senior living communities as part of FOX Rehabilitation’s Geriatric House Calls model. Deliver evaluations, individualized plans of care, and evidence-based treatment with the goal of improving mobility, independence, and functional outcomes. Document in FOX’s EMR, collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists), educate patients, caregivers, and staff to promote safety and aging in place, and optionally incorporate telehealth/virtual care to extend access.
Required Qualifications
- Valid Physical Therapy license in the state of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Current CPR certification
