Job Summary
Physical Therapist needed in Maquoketa, IA to provide one-on-one in-home and senior-living based physical therapy for older adults. Role includes evaluations, individualized care plans, evidence-based treatment, documentation in FOX EMR, collaboration with an interdisciplinary team (PT, OT, SLP, Fitness Specialists), patient and caregiver education, and optional telehealth. Employment types available include full-time in-home/senior living care, part-time with benefits eligibility, and PRN/FOX Flex with pay-per-unit. H1B sponsorship available for qualified clinicians; new graduates eligible for FOX’s Emerging Professionals Mentor Program. Required Qualifications
- Valid Physical Therapy license
- Degree from an accredited Physical Therapy program
- EMR proficiency
- CPR certification
