Job Summary
The Physical Therapist evaluates, plans, and implements individualized PT treatments for residents in a skilled nursing facility to improve mobility, strength, endurance, balance, and functional independence. Responsibilities include conducting initial and ongoing evaluations, developing treatment plans, providing therapeutic interventions (exercises, gait and balance training, mobility enhancement, manual therapy), documenting progress, educating residents and families, collaborating with nursing, occupational, and speech therapy teams, ensuring regulatory compliance (HIPAA, OSHA, Medicare PDPM), mentoring PT assistants, and contributing to quality improvement within the rehabilitation department. Requires current state licensure as a Physical Therapist and engagement in ongoing professional development to support geriatric rehabilitation and evidence-based practice.
