Job Summary
Physical Therapist to provide direct patient care in a Skilled Nursing Facility setting. Develops individualized therapy programs including physical assessment, treatment planning, implementation, and education, aimed at maximizing patient progress toward functional goals. Maintains documentation of evaluations, goals, and plans with timely updates and signatures. Collaborates with other health professionals and ancillary staff to optimize patient outcomes, adheres to standards of care, policies, and infection control requirements, and participates in ongoing professional development. Travel as needed to perform job duties. Requires effective written and verbal communication and strong organizational skills.
Required Qualifications
- Graduate of an accredited Bachelors, Masters, or Doctorate in Physical Therapy
- Current license from the state Board of Physical Therapy Examiners.
- Proficiency with all applicable state, federal, and professional standards
