Job Summary
Physical Therapist will evaluate patients, develop individualized treatment plans, and deliver interventions to restore function and reduce pain for senior residents in a Life Care Centers facility. Responsibilities include performing PT evaluations, setting functional goals, implementing therapeutic exercise programs, manual therapy, gait/balance training, and using modalities; documenting progress in EMR; communicating treatment plans and home exercises to patients and families; participating in care plan meetings and discharge planning; and upholding regulatory, safety, and infection control standards within a team-based, patient-first care culture. Required Qualifications
- Licensed Physical Therapist in applicable state
- DPT or equivalent degree (if applicable in state)
- current licensure or eligibility to practice physical therapy
- ability to evaluate patients and develop treatment plans
- experience with geriatrics or post-acute rehabilitation preferred
- strong communication and documentation skills
- familiarity with EMR systems
- ability to work within an interdisciplinary team
- commitment to regulatory, safety, and infection control standards
