Job Summary

Conduct comprehensive physical therapy evaluations using standardized assessments to determine strength, range of motion, and rehabilitation potential. Develop individualized plans of care with short- and long-term goals in collaboration with patients, families, and interdisciplinary teams. Administer skilled treatments, monitor progress, and modify plans based on physician direction and patient response. Coordinate patient discharge by assessing home readiness, recommending equipment, and connecting families with community resources. Accurately document evaluations, treatments, and outcomes while delegating to physical therapy assistants. Maintain open communication with physicians and nursing staff to ensure coordinated care across the hospital setting.

Required Qualifications

  • Licensed Physical Therapist (PT) in the applicable state
  • Graduate of an accredited Physical Therapy program
  • Strong clinical knowledge of physical therapy evaluation and treatment techniques
  • Excellent communication and interpersonal skills
  • Ability to work effectively within a multidisciplinary healthcare team
  • Commitment to quality care, safety standards, and professional ethics

Desired Qualifications

  • Prior hospital or inpatient rehabilitation experience
  • Experience working with diverse patient populations and diagnoses
