Job Summary
Provide one-on-one physical therapy to older adults in private residences or senior living communities within the FOX Rehabilitation Geriatric House Calls model. Conduct evaluations, develop individualized care plans, and deliver evidence-based therapy to improve mobility, independence, and quality of life. Document in FOX’s EMR, collaborate with an interdisciplinary team (PT/OT/SLP/Fitness Specialists), educate patients, caregivers, and senior living staff on safety and aging-in-place, and offer telehealth/virtual care when appropriate to extend access and flexibility. Eligible for multiple employment pathways (in-home, senior living, and virtual care) with compensation options and opportunities for professional growth. Requirements include a valid Physical Therapy license in the practicing state, accreditation of the PT program, CPR certification, EMR literacy, and ability to perform patient transfers and work in home/community settings.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
- Ability to perform physical tasks including lifting and transfers
- Telehealth capability is referenced as part of the role
