Job Summary
Conduct initial evaluations, daily notes, and reassessments to formulate diagnoses, goals, and treatment plans for patients. Administer tests and measures including therapeutic exercises, joint mobilization, and modalities while providing education on ADLs, transfers, and community reintegration. Synthesize evaluation results to establish and modify plans of care, oversee support staff, and initiate discharge plans. Collect and record patient care data for compliance with regulatory agencies and third-party payers, while managing daily billing and productivity.
Required Qualifications
- Bachelor's or Master's Degree
- Doctorate degree
- Licensed Physical Therapist in the state practicing in
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Basic computer skills
- Works as a team member and individually with minimal supervision
- Maintains a positive, professional demeanor at all times
