Job Summary
Conduct daily patient evaluations, administer therapeutic exercises and modalities, and execute personalized treatment plans to improve pain tolerance and functional independence. Collect and record patient care data in the Prompt EMR, including initial assessments, progress reports, and discharge plans while maintaining strict confidentiality. Synthesize evaluation results to formulate diagnoses, goals, and prognosis, then collaborate with the interdisciplinary team to modify care as needed. Provide education on activities of daily living and community reintegration to patients and their support systems. Oversee support staff including PTAs and aides, and initiate referrals to other healthcare professionals when necessary.
Required Qualifications
- Bachelor's or Master's Degree
- Licensed Physical Therapist in the state practicing in
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Basic computer skills
- Works as a team member and individually with minimal supervision
- Maintains a positive, professional demeanor at all times
