Job Summary
Conduct patient evaluations, administer tests, and synthesize results to formulate diagnoses, goals, and treatment plans. Execute daily treatment protocols including therapeutic exercise, joint mobilization, and modalities while providing education on ADLs and community reintegration. Record care data, billing, and progress reports in the Prompt EMR system. Collaborate with interdisciplinary teams, oversee support staff, and manage discharge planning. Maintain patient confidentiality and promote JAG's care-first model through community presentations.
Required Qualifications
- Bachelor's or Master's Degree
- Doctorate degree
- Licensed Physical Therapist in the state practicing in
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Basic computer skills
- Works as a team member and individually with minimal supervision
- Maintains a positive, professional demeanor at all times
