Job Summary
Conduct comprehensive patient evaluations, including initial assessments, daily notes, and reassessments to formulate diagnoses, goals, and prognosis. Establish and execute individualized treatment plans involving musculoskeletal interventions, therapeutic exercises, joint mobilization, and modality prescriptions. Administer tests and measures, prescribe rehabilitation equipment, and provide training on activities of daily living and community reintegration. Collaborate with interdisciplinary teams, oversee support staff including PTAs and aides, and initiate discharge plans. Record all care data and billing information using Prompt EMR while maintaining patient confidentiality and adhering to regulatory guidelines.
Required Qualifications
- Bachelor's or Master's Degree
- Doctorate degree
- Licensed Physical Therapist in the state practicing in
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Basic computer skills
- Works as a team member and individually with minimal supervision
- Maintains a positive, professional demeanor at all times
