Job Summary
Conduct daily patient evaluations, administer therapeutic exercises and modalities, and execute treatment plans to improve pain tolerance and functional independence. Collect and record patient care data in the Prompt EMR, including initial assessments, progress reports, and discharge plans while tracking daily billing. Collaborate with interdisciplinary teams, PTA staff, and patient support systems to modify care goals and provide education on ADLs, transfers, and community reintegration. Synthesize evaluation results to formulate diagnoses and prognoses, ensuring all interventions comply with department policies and regulatory guidelines.
Required Qualifications
- Bachelor's or Master's Degree
- Doctorate degree
- Licensed Physical Therapist in the state practicing in
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Basic computer skills
- Works as a team member and individually with minimal supervision
- Maintains a positive, professional demeanor at all times
