Job Summary

Conduct comprehensive physical therapy evaluations, treatments, and interventions for residents to improve mobility, strength, and overall function. Develop individualized treatment plans based on physician recommendations and monitor progress to adjust care as needed. Maintain accurate clinical documentation, including evaluations, progress notes, discharge summaries, and statistical records in accordance with regulatory standards. Collaborate with interdisciplinary team members to coordinate care and enhance rehabilitation outcomes. Provide input for meetings and assume Director-level supervision duties during absences, including oversight of Physical Therapy Aides. Submit requisitions for supplies and assist with in-service training programs. Requires a current State Physical Therapy License, graduation from an approved school, and two years of experience.

  • Ability to work collaboratively in a multidisciplinary team environment
  • Commitment to providing compassionate, resident-centered care
  • Graduation from an approved school of physical therapy
  • State license to practice physical therapy
  • Ability to direct others
  • Intermediate or more advanced level English language proficiency (read, write, speak, understand)
