Job Summary
As a Physical Therapist (PT) in a Home Health setting, you will evaluate and treat patients with physical disability or mobility limitations due to injury, disease, or surgery. You will establish and administer a treatment plan using exercises, stretching, manual therapy, and equipment to improve mobility and manage pain, educate patients and families in rehabilitative care and activities, and collaborate with other providers to achieve the best patient outcomes. This role supports flexible scheduling (Full-time, Part-time, PRN) and may involve 1:1 patient care in home health. Equal opportunity employer. Licensed PT with CPR/BLS/First Aid and at least 1 year of home health or similar clinical experience are required.
Required Qualifications
- Current unrestricted license to practice as a Physical Therapist (PT) in the state associated with this position.
- Current CPR/AED/BLS/First Aid certification.
- Reliable transportation to/from care sites and/or work locations
- One (1) year of professional experience practicing as a Physical Therapist (PT) in a home health or similar setting.
