Job Summary
Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury. Conduct assessments, document progress, modify care plans, and assist patients with exercises and hands-on therapy to manage pain and increase mobility. Educate patients on their plan of care and coach families on safely assisting with exercises. Work as part of a home health team including RNs, LPNs, CNAs, and OTs to support the patient's plan of care and goals. Requires a Michigan PT license, one year of clinical experience, and CPR certification. Salary ranges from $80,000 to $120,000 plus weekly bonuses.
Required Qualifications
- Graduate of an accredited Physical Therapist Program
- Active PT license in Michigan
- 1 year of clinical experience
- CPR certification
- Good clinical judgement
