Job Summary
Direct patient care, teaching, and participation on appropriate committees and departmental process improvement. Maintain primary responsibility over patient plans of care through timely collaboration with treating PTs and PTAs, while performing re-evaluations and adjusting treatment protocols based on department guidelines. Provide accurate information to patients, address concerns promptly, and document daily notes reflecting patient response, performance, and assessment status in the electronic medical record. Serves as a role model for therapists and clinical interns in professional, clinical, communication, education, and leadership activities. Contribute to continuous improvement efforts, cost reduction, and service growth through creative problem-solving and diversity in treatment adaptation. Participate in committees, special projects, and educational efforts such as in-services and journal clubs. Actively participate in student clinical programs by serving as a clinical instructor, mentor, or observation host. Flex schedule or assignment to meet patient care needs, staff meetings, and department-specific meetings outside normal work hours.
Required Qualifications
- Bachelor's Degree in Physical Therapy
- Licensed Physical Therapist in State of Illinois
- BLS from American Heart Association upon hire
- Licensed Physical Therapist who complies with the Code of Ethics/Standards of Practice as outlined by their professional organization and by the State of Illinois
- Must be available for weekend shifts per month (choice of 2 or 4)
- Must be able to lift 50 lbs
