Physical Therapist - Inpatient - Cottonwood

Conduct inpatient physical therapy assessments, treatment planning, and implementation for infants, children, adolescents, and seniors while directing support staff to achieve discharge goals. Develop individualized plans of care including home instructions, evaluate patient responses, and update protocols based on ongoing assessments. Manage patient care conferences, document progress in electronic records, and participate in discharge planning responsive to family concerns. Report safety incidents through the Midas/RDE tool, ensure compliance with state regulations and departmental policies, and attend 75% of monthly department meetings. Maintain current licensure in Arizona, BLS certification, and clinical competencies while meeting productivity targets.
