Job Summary
Physical Therapist for Home Health serving Franklin, IN area. Provides skilled PT services in patients' homes on an intermittent basis, aligns with physician orders and individualized care plans, and focuses on mobility, strength, and functional independence. Responsibilities include initial evaluations, developing and implementing individualized treatment plans, performing therapeutic exercises and manual therapy, using modalities to restore function and alleviate pain, patient and caregiver education, collaboration with physicians and the interdisciplinary team, timely documentation and reporting, participation in care planning meetings, maintaining accurate progress notes, ensuring regulatory compliance, guiding family members, contributing to QAPI initiatives, and ongoing professional development. Requires CAPTE-accredited PT program graduation, NPTE, current state licensure, and strong EMR/documentation skills; 2 years of experience preferred.
Required Qualifications
- Graduation from an accredited Physical Therapy program recognized by CAPTE
- Successful completion of the National Physical Therapy Examination (NPTE)
- Current licensure in the state(s) of practice (if applicable)
- Minimum of two (2) years of professional experience, preferred
- Proficiency in documentation and use of electronic medical records (EMR)
- Strong clinical and interpersonal skills with ability to work independently in a home health setting
