Job Summary
Physical Therapist needed for an outpatient clinic in Concord, CA. Responsibilities include performing patient evaluations, developing individualized care plans, delivering therapy services, providing daily treatments, tracking functional progress, supporting discharge planning, documenting accurately and timely, managing an outpatient caseload, and collaborating with the interdisciplinary team while ensuring patient satisfaction. Requires active California PT license (or eligibility), strong communication and teamwork, and solid documentation skills. 5x8 day shifts, typical schedule Monday–Friday 8a–5p. Opportunities to work with an active patient caseload in a collaborative rehabilitation environment. Note: facility may consider a PTA.
Required Qualifications
- Active California Physical Therapist license, or eligibility to practice in the state
- Outpatient rehabilitation experience preferred
- Strong communication, teamwork, and documentation skills
