Job Summary
Conduct thorough evaluations of patients' physical conditions and functional abilities, then develop individualized care plans addressing specific needs. Implement treatment programs utilizing exercises, manual therapy, and modalities while monitoring progress to adjust plans for optimal outcomes. Educate patients and families on rehabilitation processes, home exercises, and injury prevention strategies. Collaborate with interdisciplinary teams to ensure comprehensive care, including discharge planning for inpatient cases, and maintain accurate documentation of all evaluations and treatments. Work with clients in their homes in the Fairfax County and City area, rotating on the road.
Required Qualifications
- A degree in Physical Therapy from an accredited institution
- Strong knowledge of medical terminology and anatomy
- Must be available in Fairfax county & Fairfax city area
- Must be able to work On the road
Desired Qualifications
- Proven experience in a clinical setting, particularly in orthopedics or home community care
- Familiarity with discharge planning processes and systems used in patient care
- Excellent interpersonal skills to build rapport with patients and collaborate with healthcare teams
