Job Summary
Conduct thorough evaluations of patients' physical conditions and functional abilities, then develop individualized care plans addressing specific needs. Implement treatment programs utilizing exercises, manual therapy, and modalities while monitoring progress to adjust plans for optimal outcomes. Educate patients and families on rehabilitation processes, home exercises, and injury prevention strategies. Collaborate with interdisciplinary teams to ensure comprehensive care, including discharge planning for inpatient cases. Maintain accurate documentation of evaluations, treatments, and progress in accordance with clinic policies. Requires a degree in Physical Therapy from an accredited institution with clinical experience preferred. Located in Fairfax County and City, this role involves on-the-road home health visits.
Required Qualifications
- A degree in Physical Therapy from an accredited institution
