Job Summary
Conduct direct patient care, including initial evaluations, re-evaluations, and discharge planning for functional progress and safety. Manage patient plans of care through consistent collaboration with treating PTs and PTAs while documenting responses to treatment and status in the electronic medical record. Serve as a clinical instructor, mentor, or observation host for student programs and participate in department committees and continuous improvement projects. Maintain primary responsibility over patient management from initial contact to discharge, ensuring accurate documentation supports reimbursement and payer interpretation. Demonstrate essential skill competencies in selecting appropriate evaluation and treatment modalities, adjusting protocols based on evidence-based team feedback.
Required Qualifications
- 2 weekend shift per month
- 4 weekday hours per month
- holiday rotation
- Bachelor's Degree in Physical Therapy
- Licensed Physical Therapist in State of Illinois
- BLS from American Heart Association upon hire
- compliance with the Code of Ethics/Standards of Practice as outlined by their professional organization and by the State of Illinois
- adherence to the organization's Code of Ethics and Corporate Compliance Program
- compliance with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards
- willingness to flex schedule or assignment to meet patient care needs, staff meetings and department specific meetings outside of normal work hours
- completes authorization and disclosure form for background check
