Physical Therapist I - Rehab

Evaluates and treats patient populations for therapy needs. Performs and documents initial and ongoing assessments of patient's condition. Establishes a plan of care appropriate to identified problems, involving the patient and family according to their capabilities and desires. Provides education to the patient and family/caregiver about deficits and use of adaptive equipment and support programs. Plans and/or assists with discharge from therapy services and treatment setting. Supervises support personnel within licensing and practice standards. Requires critical thinking, decisive judgement, and the ability to work with minimal supervision in a fast-paced environment. Onsite role at UCHealth Harmony Campus in Fort Collins, CO.
