Required Qualifications
- Graduate from an accredited school of physical therapy
- Georgia PT license or willing to obtain temp license
- Minimum 1 full year Adult Home Health Experience
- Performs patient evaluations and re-evaluations and documents in accordance with licensure, scope of practice and department standards
- Advocates for and is active in developing disciplinary and interdisciplinary team members.
- Ensures they are able to go anywhere in the house as needed.
- Verifies and reviews all provider orders and applies all pertinent medical information including lab values, vital signs, etc. routinely.
- Demonstrates knowledge and understanding of the profound and prolonged impact of chronic illness.
