Job Summary
Physical Therapist (PT) for Homecare delivering patient-centered, age-appropriate evaluation and treatment plans based on physician referrals and patient needs. Responsibilities include conducting assessments, planning and implementing individualized treatment programs, coordinating care with families and health professionals, supervising Physical Therapist Assistants, and maintaining thorough documentation. Travel to patient homes within the New River Valley coverage region, ensuring regulatory compliance, providing team-based care, promoting cost-effective care, and contributing to program development and performance improvement. Requires VA licensure, a degree in physical therapy (BS, MS, or DPT), and BLS within six months of employment. May require travel between Carilion facilities and patient homes; mileage reimbursement provided.
Required Qualifications
- Graduate of an approved school of Physical Therapy
- Bachelor's degree as Physical Therapist and 5 years of experience, OR Master's or Doctorate degree required
- Licensed to practice in the Commonwealth of Virginia by the Virginia Board of Physical Therapy
- Life Support: AHA BLS-certified (within six months of employment) and BLS-certified (within six months of employment)
