Job Summary
Skilled Home Health Physical Therapist (PT) delivering patient-centered care in Northern Virginia. Conduct comprehensive assessments, design and implement individual treatment plans to improve mobility, strength, and independence; provide hands-on therapeutic exercises; educate patients and caregivers for at-home care; track progress and adjust plans; collaborate with an interdisciplinary care team; maintain accurate documentation to meet clinical and compliance standards. Requires a Master’s or Doctorate in Physical Therapy, active Virginia PT license and CPR certification, 2+ years of experience (home health preferred), and strong communication and compassionate, patient-first approach. Sign-on bonus $5,000; full-time role in Northern Virginia with flexible scheduling within home health settings.
Required Qualifications
- Master's or Doctorate degree in Physical Therapy (from an accredited program)
- Active Virginia PT license
- CPR certification
- 2+ years of experience as a Physical Therapist (home health preferred but not required)
- Experience with Home Care Home Base (HCHB) is a plus
