Physical Therapist - Home Healthcare

Physical Therapist provides and supervises primary rehabilitative care in patients' homes for ~25-35 individuals, coordinating with RNs/LPNs, Therapists, Social Work, Home Health Aides, physicians, and families. Responsible for developing and implementing the plan of care, supervising PTAs and Home Health Aides, completing OASIS and EMR documentation, and participating in performance-improvement activities. Requires strong clinical skills, leadership, communication, and knowledge of CMS/State/Joint Commission guidelines; prior home health experience and BLS certification preferred. Must be graduate of an accredited physical therapy program with current licensure and one year of full-time physical therapy experience.
