Physical Therapist - Home Healthcare

Physical Therapist - Home Healthcare provides and supervises rehabilitative care for 25-35 patients in the home setting, coordinating care across RNs, LPNs, Therapists, Social Workers, Home Health Aides, and physicians. Responsibilities include developing and supervising the plan of care, scheduling visits per physician orders, supervising PTAs and Home Health Aides, ensuring regulatory compliance (CMS, Joint Commission), and delivering patient/caregiver education to promote independence. Requires strong clinical assessment, leadership, communication, and adherence to home health operations and documentation in the EMR, with a focus on improving patient outcomes and agency performance.
