Job Summary
Physical Therapist leading home healthcare services in the LaGrange, GA area, providing and supervising rehabilitation for patients in their homes (approximately 25-35). Develops and implements patient plans of care per physician orders, coordinates with RNs, LPNs, therapists, social workers, HHAs, and the physician; supervises PTAs and Home Health Aides; educates patients and caregivers to promote independence; ensures regulatory and documentation compliance (OASIS, CMS/state guidelines) and participates in performance improvement initiatives. Requires leadership, strong communication, clinical assessment, and adaptability to home-based rehab settings.
Required Qualifications
- Graduate of an accredited school of physical therapy
- Minimum 1 year of full-time Physical Therapy experience
- Home Health experience preferred
- Basic Life Support (BLS) - Instructor preferred
- Georgia Driver's License
- Private Vehicle insurance
- Ability to write and follow directions to patients' homes
- Strong communication and organizational skills
- OASIS knowledge preferred
- Knowledge of CMS/State regulatory guidelines
- Experience with documentation in EMR and visits scheduling
