Physical Therapist - Home Healthcare (Lagrange, GA)

Physical Therapist provides and supervises primary care in the home setting for 25-35 patients, coordinating rehabilitative care and collaborating with RNs, LPNs, therapists, social workers, home health aides, and physicians. Core duties include developing and supervising patient plans of care, performing routine and complex therapy procedures, coordinating episode-of-care services, supervising PTAs and home health aides, ensuring timely documentation and communication, educating patients and caregivers to promote self-care, and participating in performance improvement activities. Requires knowledge of CMS/State/Joint Commission guidelines and completing OASIS documentation; weekend and call rotations are required.
