Physical Therapist - Home Healthcare

Physical Therapist - Home Healthcare needed to provide and supervise primary care for 25-35 patients in home settings. Responsibilities include direct patient care, coordinating with RNs/LPNs/therapists/social workers, supervising PTAs and Home Health Aides, developing and adjusting plans of care, educating patients and caregivers, participating in performance improvement, and fulfilling CMS/Joint Commission regulatory requirements. Must maintain documentation in EMR, manage visits and scheduling per physician orders, and participate in weekend and call rotations. Graduate of an accredited physical therapy program with ongoing clinical knowledge and strong communication skills required. Based in La Grange, GA with in-home patient visits.
