Physical Therapist - Home Health Visits (PRN)

Job Summary
Provide Physical Therapy in a home health setting: evaluate and treat patients with mobility limitations due to injury, disease, or surgery; establish and administer treatment plans using exercises, stretching, manual therapy, and equipment to improve mobility and reduce pain; educate patients and families in rehabilitative care to promote health, safety, and independence; collaborate with other care providers to achieve optimal patient outcomes; requires active PT license in state, CPR/BLS/First Aid certification, reliable transportation, and 1 year of home health experience. Part-time/PRN role with flexible scheduling and 1:1 patient care; competitive pay and benefits.
Required Qualifications
- Current unrestricted license to practice as a Physical Therapist (PT) in the state associated with this position.
- CPR/AED/BLS/First Aid certification.
- Reliable transportation to/from care sites and/or work locations
- One (1) year of professional experience practicing as a Physical Therapist (PT) in a home health or similar setting.
