Job Summary
Physical Therapist (PT) in home health visits (PRN, part-time) evaluating and treating patients in home care. Establishes and administers a treatment plan using exercises, stretching, manual therapy, and equipment to manage pain and increase mobility; educates patients and family members in rehabilitative care; collaborates with other providers to achieve optimal patient outcomes; role supports 1:1 patient care in a home health environment; requires an active PT license, CPR/BLS/First Aid certification, and reliable transportation to care sites. Required Qualifications
- Current unrestricted license to practice as a Physical Therapist (PT) in the state
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- One (1) year of professional experience practicing as a Physical Therapist (PT) in a home health or similar setting
