Job Summary
Physical Therapist (PT) needed for Home Health visits, PRN/Part-time. Evaluate and treat patients in home care, establish and administer treatment plans using exercises, stretching, manual therapy, and equipment to manage pain and increase mobility. Educate patients and families in rehabilitative care and activities to promote health, safety, and independent living. Collaborate with other providers to achieve optimal patient outcomes. Requires active PT license, CPR/BLS/First Aid certification, reliable transportation, and 1 year of professional PT experience in home health or similar setting.
Required Qualifications
- Current unrestricted license to practice as a Physical Therapist (PT) in the state associated with this position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- One (1) year of professional experience practicing as a Physical Therapist (PT) in a home health or similar setting.
