Physical Therapist - Home Health Visits (PRN)

Job Summary
Provide Physical Therapist (PT) services in a home health setting: evaluate and treat patients with mobility limitations, establish and administer treatment plans using exercises, stretching, manual therapy, and equipment; educate patients and families on rehabilitative care to promote health and independence; collaborate with other providers to optimize patient outcomes; requires current PT license in the state, CPR/BLS/First Aid certification, reliable transportation, and at least one year of home health or similar experience. Part-time/PRN role with 1:1 patient care and flexible scheduling.
Required Qualifications
- Current unrestricted license to practice as a Physical Therapist (PT) in the state associated with this position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- One year of professional PT experience in a home health or similar setting
