Physical Therapist - Home Health Visits (PRN)

Job Summary
Deliver in-home physical therapy assessments and treatment plans for patients with mobility limitations; perform exercises, manual therapy, and use equipment to improve mobility and reduce pain; educate patients and families on rehabilitative care to promote health, safety, and independent living; collaborate with other providers to optimize patient outcomes; requires an unrestricted Physical Therapist license in the applicable state, CPR/BLS/First Aid certification, reliable transportation, and at least 1 year of experience in home health or similar settings. Part-time/PRN schedule with 1:1 patient care in a home care environment.
Required Qualifications
- Current licensure to practice as a Physical Therapist (PT) in the state of the position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites/work locations
- One (1) year of professional experience as a Physical Therapist (PT) in home health or similar setting
