Physical Therapist - Home Health Visits (PRN)

Job Summary
Provide physical therapy services in a home health setting including evaluating and treating patients with mobility impairments, establishing and administering treatment plans using exercises, stretching, manual therapy, and equipment to reduce pain and improve mobility, and educating patients and families on rehabilitative care to promote health, safety, and independence. Collaborate with other providers to optimize patient outcomes. Requires current PT licensure, CPR/BLS/First Aid certification, reliable transportation, and at least 1 year of PT experience in home health or similar settings. Flexible Part-time/PRN schedule with daily pay option; equal opportunity employer.
Required Qualifications
- Current unrestricted license to practice as a Physical Therapist (PT) in the state associated with this position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- One (1) year of professional experience practicing as a Physical Therapist (PT) in a home health or similar setting
