Job Summary
Physical Therapist (PT) to provide in-home, one-on-one care in a PRN/part-time home health setting. Responsibilities include evaluating and treating patients with mobility impairments, establishing and administering a treatment plan using exercises, stretching, manual therapy, and equipment; educating patients and family members on rehabilitative care to promote health and independence; and collaborating with other providers to achieve optimal outcomes. Requires current PT licensure, CPR/AED/BLS/First Aid certification, reliable transportation, and at least 1 year of home health or similar experience. Emphasizes flexible scheduling and patient-centric care.
Required Qualifications
- Licensure: Current unrestricted license to practice as a Physical Therapist (PT) in the state associated with this position.
- Current CPR/AED/BLS/First Aid certification.
- Reliable transportation to/from care sites and/or work locations
- One (1) year of professional experience practicing as a Physical Therapist (PT) in a home health or similar setting.
