Job Summary
Conduct home health visits to adult and geriatric clients in Carroll County, Maryland, performing diagnostic tests for mobility, strength, balance, and safety assessments. Implement integumentary protective interventions and develop individualized care plans including manual therapeutic exercises, gait training, and balance interventions. Continually assess and revise care plans while collaborating with nursing staff, educate clients and families on rehabilitative activities, and accurately document observations using a touch pad tablet. Requires a current Maryland Physical Therapist license, one year of recent experience, and proficiency with electronic medical records. Full-time, part-time, or per diem availability between 8:30a-5p Monday-Friday.
Required Qualifications
- A current Maryland Physical Therapist - PT license.
- A minimum of one year of recent work experience as a Physical Therapist - PT.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
Desired Qualifications
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
